The transition back into the community and returning to a full range of activities after an acquired brain injury (ABI) can be difficult for individuals and their families. Our community ABI rehabilitation serves clients within their local community across the Hamilton, Burlington, Brant, Haldimand and Norfolk regions.

Through our client-centered application process, we get to know the needs and goals of each individual applying for Outreach and Group services. The first step is understanding the eligibility criteria and completing an admissions application. Through the conversations that follow, we will work together to establish an individualized rehabilitation plan to facilitate transition to a higher level of independence.
