Finding the right brain injury services and supports can be overwhelming. HIRO’s system navigation team helps individuals living with an acquired brain injury (ABI), along with families, caregivers, and professionals, navigate health and social services and connect to resources and supports across Ontario. Whether you are looking for community programs, service coordination, or guidance on where to start, we are here to help.
How HIRO Can Help
HIRO’s system navigation team supports individuals living with ABI, families, caregivers, professionals, and community partners by:
- Helping individuals and families access appropriate brain injury services and resources
- Assisting caregivers and professionals navigating health and social service systems
- Coordinating complex cases and supporting collaborative care planning
- Helping professionals, caregivers and those with lived experience make connections
- Linking service providers to ABI-related resources and information
- Educating community organizations and system partners about the brain and acquired brain injury
- Participating in system planning initiatives to improve services and supports for people with ABI
- Collaborating with allied agencies and partners to strengthen system capacity and influence ABI-related policy
Medical and social services systems can be complicated. We work with community organizations, healthcare providers, caregivers, and individuals living with ABI to access services by acting as guides and advocates and strengthening connections across the system.
