HIRO is unique in its ability to provide evidence-based, client and family centered ABI rehabilitation care in the community. We work with clients and their families to achieve goals that are meaningful for them throughout our continuum of care; from six residential care homes, to transitional living apartments and to community services including group as well as outreach. HIRO serves people from across Ontario through services in Hamilton, Burlington, Niagara, St. Catharines, Brant and Haldimand-Norfolk regions.
RESPONSIBILITIES:
As a case facilitator you will participate in the development of individualized rehabilitation plans designed to promote respect, learning and independence. A case facilitator also coordinates the service delivery process for assigned clients.
MINIMUM QUALIFICATIONS:
Completion of post secondary education in a related field; the equivalent of two to three years related work experience (3900 – 5850 hours) with a background in goal planning, teaching skills and establishing therapeutic rapport. Participate in the development of individualized rehabilitation plans designed to promote respect, learning and independence.
The candidate must also have good organizational skills and excellent verbal and written communication and must be able to take clinical direction. The successful candidate will be a team player who is able to work independently. A high degree of self-motivation, flexibility, and initiation are a must.
- Access to a reliable vehicle and a valid driver’s licence is required
- Valid First Aid/CPR certificate Level ‘C’
- Current criminal reference check with vulnerable screening
- Ability to speak French is an asset
